Prescription for Life

The Gladstone Public Safety Department's Prescription for Life program is a service provided to Gladstone residents to aid emergency medical personnel in obtaining vital patient health information in the event of a medical emergency.  When emergency personnel see the Prescription for Life sticker on your front door, they know where to go in your house to find your medical information when you are unable to provide it.   This program is designed to save time in situations when every second counts.

For the program to work successfully, it is important to complete all of the following steps.

• Obtain a Prescription for Life bottle for each family member.

• Fill out the sticker label with the person's name and place on the outside of the bottle.

• Complete a medical information sheet for each family member and place it in the appropriate bottle.

• Place the Prescription for Life bottles in the refrigerator on the top shelf, towards the rear, so they can be found easily.

• Place the Prescription for Life sticker on the front door of your residence where it can be easily seen.

• Update the medical information sheets when any changes occur.
